804 Audit of Hand-Written Consent Form Documentation for Skin Lesion Excisions in the Plastic Surgery Procedure Room

Abstract Aim To audit the hand-written consent forms for elective excision of skin lesions in Procedure Room. Method The notes and from 11 Room sessions were analysed. total number reviewed 24. Our standards focused on consenter completion sections form. There are no standard guidelines complications lesion excision, therefore a consensus general consultant body was used. Results patient demographic mostly consisted male patients (15, 63%) with mean age 50 (range 17 – 75). 83% procedures required reconstruction, 8% biopsies full thickness graft (FTSG). 75% consenting clinicians Registrars. rest Senior House Officers. All consenters documented their job title, signed dated signature. trained each performed procedure. 100% used correct form, proposed procedure, clearly site risks placed label every page. completed ‘intended benefits’ section. 58% wrote procedure name full. Every clinician bleeding infection as risk factor. 54% pain, re-excision margins, 71% recurrence wound breakdown, 96% scarring, 46% injury to surrounding structures 0% anaesthetic risk. who received FTSG, had loss Conclusions quality is user dependent. A pre-filled form increase compliance.
